HORSE INTAKE FORM

We’re excited to welcome you and your horse to Mustang Alley Ranch! This Intake Form helps us provide the best care possible for your horse while they’re with us. Please provide detailed and accurate information about your horse’s health, care preferences, and emergency contacts. Full-Care Board: $1,200/month

Our all-inclusive boarding option. We provide a grassy paddock, 24/7 access to custom run-in shelters, automatic waterers, free-choice salt blocks, use of our 2 large riding arenas, plus all of the premium "A La Carte' services:

Hay - we provide & distribute 5' quality round bales
Grain - we provide a locally-grown 12% blend & feed 2x per day
Fly Spray - we apply the product of your choice daily (boarder-provided)
Supplements - we administer to your instructions (boarder-provided)
Deworming - we provide & administer quarterly:
- March - Equimax (Praziquantel + Ivermectin)
- June - Zimecterin (Ivermectin)
- Sep - Quest Plus (Moxidectin + Praziquantel)
- Dec - Panacur (Fenbendazole)
Trace Mineral Blocks - we provide 50-lb blocks & replace as needed
Paddock Cleaning - we provide weekly manure removal from paddock & run-in area
Basic Grooming - total body brushing, mane/tail combing, hoof picking (2x/month)
Farrier Holding - just confirm with us the day & time of your appointment
Veterinarian Holding - just confirm with us the day & time of your appointment
Tack Locker - private use of a key-locked storage shed (5' x 3' x 5' - 75 cu ft)
Trailer Parking - secure spot to park your horse trailer on-site

Self-Care Board: $600/month

Hay:

I will supply and feed hay.

$120/month - Stable supplies & feeds hay (5' round bales).

Grain Supply:

I will supply and feed grain.

I will supply grain for the Stable to feed my horse.

Boarder may purchase Stable-Provided grain for $20/bag (a local 12% Maintenance Blend) or purchased elsewhere and dropped off to the Stable.

I will deworm regularly and provide proof to Stable upon request.

$10/month - My horse will remain on Stable's Deworming Schedule.

If opted into our deworming schedule, we will provide and administer quarterly. Annual fee is billed in monthly installments of $10/month:

March - Equimax (Praziquantel + Ivermectin) - strongyles emerge, bots, tapeworms

June - Zimectin (Ivermectin) - pinworms peak, roundworms, strongyles

Sep - Quest Plus (Moxidectin + Praziquantel) - larval strongyles, tapeworms, bots

Dec - Panacur (Fenbendazole) - roundworms, pinworms, encysted strongyles

Basic Grooming (body brushing, mane/tail combing, hoof picking):

$50/month (Stable grooms monthly)

Paddock Cleaning:

I will move manure to corner of paddock for weekly disposal.

$200/month - Stable will keep my horse's run-in and paddock clean.

Trace Mineral Block (Salt blocks are already provided by stable, free of charge):

Trailer Parking:

$40/month (on-site parking)

Tack Locker Storage:

$20/month (private key-locked shed)
